Rehabilitation Services
SMMC has been providing exceptional outpatient rehabilitation services for more than 25 years. We believe that one of the key components to a successful recovery is a team approach, with patients taking an active role in their overall therapy plan and defining and redefining their goals as they progress. An individualized approach allows our team the ability to customize a program for each patient, with careful assessment, monitoring, and a steady flow of encouragement at every level along the way. SMMC offers rehabilitation services at two convenient off-site campuses: We work closely with the Northern York County YMCA and the Partnership for Healthy Aging to offer additional fitness and wellness benefits to those individuals who may need or desire supplemental activities for their recovery plan or for those interested in seeking or maintaining additional activities to promote or maintain a healthier, independent lifestyle.

  • CWOCN (Certified Wound, Ostomy, Continence Nurses)
