Welcome to mySMMCchart, the SMMC PrimeCare Physicians Patient Portal

my SMMC chartWe know you’re busy so we have introduced a new online service, called mySMMCchart, to help SMMC PrimeCare patients simplify their lives. This secure and confidential web-based patient portal allows you to:

  • view your personal physician office health record;
  • request non-urgent appointments;
  • request medication refills;
  • request a referral; and,
  • update personal demographic information (address, preferred pharmacy, etc).

Some frequently asked questions (FAQs) regarding mySMMCchart:

  • Is this patient portal secure?
    Yes, mySMMCchart follows all the HIPAA guidelines (Health Insurance Portability and Accountability Act), and is secure and confidential.
  • What internet browser does mySMMCchart support?
    Internet Explorer is the recommended and preferred browser; however, mySMMCchart will run on most browsers. Please note that if you are not using Internet Explorer as your default browser, you may run into issues viewing screens and accessing your information.
  • Does mySMMCchart work on iPads and Apple notebooks and desktops?
    mySMMCchart is a Windows-based application. It is recommended and preferred that you access mySMMCchart from a Windows-based computer; however, you can access mySMMCchart from an Apple-based device. Please note that if you are not using a Windows-based computer, that you may run into issues viewing screens and accessing your information.
  • How do I establish a password?
    When your practice creates your mySMMCchart account, you will receive a welcome email that includes a link and your temporary password. Prior to gaining access to mySMMCchart, you will be required to validate your identity by answering one of the two validation questions. Once you have successfully validated your identity, you will be prompted to change your password as well as pick or create a security question. This question will be saved to your account in the event that you forget your password or need to create a new password. We recommend that you pick a question or create your own question that you can easily remember as mySMMCchart does not supply hints to these questions should you forget the answer.
  • I have forgotten my password, how do I reset it?
    From the login screen, click on the "forgot password" link and follow the instructions. An email will be generated and sent to your email account on file that will include a reset password link.
  • I reset my password using the “forgot password link” but I am not able to login:
    mySMMCchart needs time to synchronize with the database. This may take up to sixty minutes.
  • I’m getting a message telling me that my login failed and that I have two more attempts to login:
    Either the user name or password you are entering is incorrect. If you enter your user name or password incorrectly two more times, your account will be automatically locked by the system. To have your account unlocked, you will need to contact the mySMMCchart helpline. If you need to verify your user name, for any reason, you will need to call the helpline at 283-PORT (7678). The helpline staff will need to verify who you are and will need to ask you several questions prior to giving you your username. We do not have access to your password but you can reset your password by clicking on the “forgot password” link at the login screen. Please note that you can call the helpline at anytime and leave a voice mail however; the helpline hours of operations are 9:00am to 1:00pm, Monday through Friday. If you leave a message outside of the hours of operations, we will return your call during operating hours.
  • I am getting a message telling me that my account has been locked. What does that mean?
    The system allows three incorrect login attempts. If you go exceed three attempts, your account is locked automatically and you will need to call the helpline at 283-PORT (7678) to have the account unlocked. Please note that you can call the helpline at anytime and leave a voice mail however; the helpline hours of operations are 9:00am to 1:00pm, Monday through Friday. If you leave a message outside of the hours of operations, we will return your call during operating hours.
  • When I enter my user name, the password field automatically adds a series of characters, (X). (Note: These instructions vary depending on what version of Internet Explorer you have):
    This means that you have allowed Internet Explorer to save your password which we do not recommend. To clear your password, from Internet Explorer go to tools, internet options, delete browsing history and click delete passwords.
  • Can I access my spouse’s mySMMCchart account?
    No, accounts are secure and confidential. They are set up based on a patient’s chart and require a unique user name. We do not recommend you share your user name or password with anyone.
