SMMC Visiting Nurses Awarded National Diabetes Accreditation
SMMC Visiting Nurses is proud to announce that its Diabetes Education Program has been awarded national accreditation by the American Association of Diabetes Educators (AADE). The program was previously accredited by the American Diabetes Association and chose to seek re-accreditation through AADE because of its greater flexibility and alignment with the multiple sites the SMMCVN program offers education throughout the community. Medicare, Medicaid and private insurers recognize both accrediting bodies equally for their strict adherence to assuring high quality and professional practice of the organization that receive their approval.

Deb Dumoulin, RN, RD (SMMCVN Diabetes Educator) and her colleagues who met the strict requirements for recognition were told by the surveyor at AADE that the application and accompanying documentation submitted was "the best, most comprehensive application that they had received thus far." Leslie E. Kolb, RN, BSN, MBA, Program Director of the Diabetes Education Accreditation Program went on to say, "Trends show that diabetes education is moving out of the hospital and into the community. SMMC Visiting Nurses Diabetes Education Program is exactly the type of program we envisioned when we set up our accreditation program in 2009."

Diabetic education programs offered by SMMC Visiting Nurses include:

  • Grocery Store Tours - learn how to read labels & plan your meals

  • Refresh for Success - a great “tune-up” in diabetes management

  • The Sweetest Loser - a realistic weight loss program

  • Classes- Diabetes Self Management & Training - with others who have diabetes

  • Meal Planning - one on one counseling with a Dietitian

  • Meter Training - help with blood sugar measurement

  • Injection & Pump Training - we can help with your Insulin

  • Gestational Diabetes - meter training & meal planning when you’re pregnant

  • Pre-Diabetes Classes- what you don’t know can hurt you

  • Support Groups- learn from others & share info

  • Telephonic Diabetes Education – counseling and help by phone

  • Public Info – speaking to community groups
