| Weekday availability with 1 weekend per month
Community Health Nurse (RN)provides direct patient care to patients actively involving family/friends or significant others. Assesses the patient's and family's physical, emotional, and spiritual needs. Appraises existing and potential health hazards for the individual and family. Provides health counseling, including emotional support, to individuals, families and caregivers. Consults with and refers patient and caregiver to appropriate personnel within the agency, school, and/or other community services or resources. Provides services which reflect the Agency philosophy and standards of home health and nursing care of assigned patients. Completes documentation and chart review in a timely manner to comply with Agency standards. The Registered Nurse reports to and is evaluated by the Clinical Manager.

| Qualifications: Graduate of a state approved School of Nursing or Bachelor of Science in Nursing program.
Current license to practice as a Registered Professional Nurse in the State of Maine.
One to two years of clinical experience in an acute care setting. One year of home health experience preferred. |
